How Leeds implemented and spread Personalised Care and Support Planning

The Leeds team in West Yorkshire and Harrogate Demonstrator site have produced a video that describes their journey in implementing PCSP throughout primary care and describes lessons learned on that journey. Person centred dementia care

Health Education England has released a new film highlighting the importance of person-centred care in enabling people in care homes to live well with dementia.  Finding Patience – the later years continues to follow Patience and her family, who we were introduced to in the film ‘Finding Patience’ as she moves into a care home.  The film explores the challenges faced by staff and demonstrating what good quality person-centred care looks like. 

Person centred care for people with Diabetes

The Care Quality Commission has published My Diabetes My Care. This review considers how well care services work together to deliver high-quality diabetes care.  Pages 25 to 30 describe the importance of person centred care, care planning and care coordination in supporting and improving diabetes care and prevention.
