Refer To Practice - PharmOutcomes Form (This form is NOT intended to be given to the patient. Access to a PC is not always available when speaking to a patient and so this form can be used during a Minor Ailments, Care Navigation or Support for Self-Care consultation prior to transferring the entry into PharmOutcomes. A referral notification letter can either be printed off or emailed directly to the practice.)
Refer to Practice - Patient Form (This form can be given to the patient as a means of communicating with the surgery following a Minor Ailments, Care Navigation or Support for Self-Care consultation.)
The following are examples of Notification Letters that can either be printed off or emailed automatically to the surgery by PharmOutcomes (provided that the system has an email address linked to the surgery.)
